Dialysis Social Worker Employer: M2 Healthcare Ltd. Location: Brooklyn Gardens, Brooklyn, NY Base pay range: $40.00/hr - $45.00/hr Brooklyn Gardens Dialysis is seeking a social worker! At Brooklyn Gardens Hemodialysis, we are committed to improving the quality of life for patients with end‑stage renal disease (ESRD) and chronic kidney disease (CKD). Our center is staffed by board‑certified nephrologists and dedicated nursing personnel who are committed to patient comfort, health, and well‑being. Responsibilities Conduct psycho‑social assessments on each patient and develop a social work treatment plan compatible with the overall medical treatment plan. Be knowledgeable of benefits available to patients under Public Law 92‑603, Section 2991 Medicare, Medicaid, Blue Cross, and other insurance coverage. Assist patients with interpreting benefits and help them obtain the benefits to which they are entitled. Participate in multidisciplinary conferences and long‑term, short‑term care planning of the patient. Utilize crisis‑intervention techniques according to assessed individual or family needs. Use psychiatric consultation in selected situations. Refer patients and families to appropriate public and private community agencies and assist with transportation, public assistance, vocational rehabilitation, etc. Liaise between the Dialysis Center, affiliated hospitals, Brooklyn Gardens Nursing Home, and other dialysis centers; collect pertinent information for patient transfer or transient visits. Requirements Must have experience working with dialysis patients. Master’s degree in Social Work with a specialization in clinical practice from an accredited School of Social Work and certification from the New York State Education Department. Must be licensed as a social worker. Qualified by credentials and experience to meet federal regulations for Social Workers in ESRD programs.
